Our commitment to you as your partner in care is to help you receive all applicable benefits from your dental insurance plan. To that end, we will need to have accurate information regarding your plan and your existing dental work. Please be aware that your insurer may require us to submit pre- and post-treatment images of the teeth we treat. We ask that you provide the details of your dental insurance plan at least two business days prior to your appointment, to allow us to estimate insurance benefits as accurately as possible.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. If your insurance benefit is less than the amount estimated, we will send you a statement for the balance. Your prompt payment is appreciated.
