Welcome to Triangle Restoration Dentistry! We are always happy to have new patients, and our goal is for your first visit to be one of comfort and acknowledgment of your personal dental needs. We will take the time to gather all necessary patient information and answer any questions or concerns you may have.

We will discuss your immediate and long-term dental concerns, review your past medical and dental history, complete a comprehensive dental evaluation, and create a treatment plan that optimizes your dental health.

What to Expect for New Patients

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done on the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any x-rays, if applicable.
  • A list of medications you are currently taking.
  • If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

What Kind of New Patient Information Will I Need to Provide?

We will need several pieces of new patient information in addition to our initial assessment.

Pre-Existing Medical Conditions

Please alert the office if you have a medical condition that may be of concern prior to surgery, such as:

    • Diabetes
    • High blood pressure
    • Artificial heart valves
    • Artificial joints
    • Rheumatic fever
    • Autoimmune disorders

If you have these or any other medical conditions related to mental or physical health, please inform our staff.

Medications

As new dental patients, it is essential to disclose all medications or supplements being taken before any procedures are performed. Some examples of medications or supplements include:

    • Heart medications
    • Anticoagulant therapy (i.e., blood thinners)
    • Aspirin
    • Nutritional supplements (i.e., vitamins, fish oil)

It is important to disclose all medications or supplements before any procedures are performed.

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office for your consultation. If additional films are necessary, they can be taken at our facility.
